A project has been jointly funded by Cancer Australia and Hume RICS to develop a model of Care Coordination for Older Australians with Cancer (COAC). The aim of this project is to improve care coordination and supportive care for elderly cancer patients through the development of a system for routine screening of oncology patients over 70 years of age to identify their supportive care needs.
In collaboration with the Project Coordinator, the Cancer Care Coordinator will be responsible for the implementation of a model of care coordination for older patients with cancer which includes conducting routine screening of patients to determine their supportive care needs. The Cancer Care Coordinator will liaise with specialist oncology and geriatric services, as well as primary care services, to ensure appropriate referral of patients to services to create an environment for enhanced care provision.
Along with the Project Coordinator, this position will be based with the Hume RICS Border/East Hume Cancer Network team located in Wodonga. This is a full time fixed term position until June 2011.
As a part of a range of cancer reforms and guided by the Cancer Services Framework for Victoria, nine integrated cancer services (ICS) were formed across Victoria. Hume RICS is one of five rural, three metropolitan and one state-wide Paediatric ICS. The key priority areas for cancer services improvement identified for the Victorian ICS initiative include:

This role will support Occupational Therapists through advocacy, role modelling and supervision to ensure that their specific skills and contributions are recognised as individuals or as members of a multi-disciplinary team.
The position will also be responsible for providing Occupational Therapy services within the Specialist Residential Rehabilitation Program (SRRP) and Prevention and Recovery Care (PARC) services. PARC is a new 10 bed short stay facility with a focus on providing sub acute step up or step down care from the community or acute mental health in-patient unit. SRRP is also a 10 bed facility located adjacent to PARC and provides medium to long tern psychosocial rehabilitation to clients recovering from a serious mental illness.
The incumbent will provide both direct client assessment and treatment planning, as well as work closely with program staff to implement agreed treatment strategies. This may include the provision of both formal and informal staff training around subjects like ADL assessment and program planning. There is also the potential to support the development of group programs within the services.
